~~~ Cullman City Head Start Registration ~~~

The Cullman City Head Start program is located at Cullman City Primary School. The program accepts applications all year. Applications are taken by appointment only. Enrollment is open to all children, regardless of race, sex, creed, color, national origin, or disabling condition, who are three or four years of age on or before September 2nd of the program year, and who meet the eligibility guidelines.

The parent must bring the following documents in order to register his/her child:

1. Child's state certified birth certificate (hospital certificates do not qualify)
2. Proof of income. You must provide proof of any child support that is received, plus one of the following:


a.) Individual Income Tax Form 1040
b.) W-2 Forms
c.) A dated and signed verification of income from your employer
(Head Start will provide you with a "Verification of Income" form upon request.)
d.) Documentation showing current status as recipients of public assistance.



3. Social Security cards for everyone in the household
4. Child's Medicaid card or private insurance card
5. State of Alabama Certificate of Immunization (blue slip from the Health Department)
6. Food Stamp case number
7. Proof of residence (utility bill showing your name and address)
8. Child custody agreements (if applicable)
